We are currently seeking compassionate and dependable PRN Licensed Practical Nurses (LPNs) to join our team. Position Summary
As a PRN Home Health LPN, you will provide skilled nursing care to patients in their homes under the direction of a Registered Nurse and in accordance with physician orders. This role offers a flexible schedule while allowing you to build meaningful relationships with patients and their families.
Responsibilities
- Provide skilled nursing visits in patients' homes in accordance with the plan of care.
- Perform nursing assessments and report changes in patient condition to the supervising RN.
- Administer medications and treatments as ordered by the physician.
- Educate patients and caregivers on disease management, medications, and safety.
- Complete accurate and timely documentation in the electronic medical record.
- Collaborate with physicians, case managers, therapists, and other interdisciplinary team members.
- Ensure compliance with Medicare Conditions of Participation and agency policies.
- Current Florida Licensed Practical Nurse (LPN) license in good standing.
- 1 year of nursing experience preferred.
- Home health experience is preferred but not required.
- Current BLS/CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Strong communication, organization, and time management skills.
- Ability to work independently while providing compassionate, patient-centered care.
