Territory: Micco/Barefoot Bay, FL

Position Overview:
We are seeking a dedicated Field Registered Nurse (RN) to provide skilled nursing services to patients in their homes. The RN will assess, plan, and implement individualized care to ensure optimal patient outcomes and compliance with physician orders and Medicare guidelines.
Responsibilities:

Perform comprehensive patient assessments in the home setting

Develop and update individualized plans of care based on patient needs and physician directives

Provide skilled nursing services, including wound care, medication administration, and disease management education

Conduct OASIS Start of Care, Recertification, Resumption, and Discharge visits

Communicate effectively with physicians, patients, caregivers, and the interdisciplinary team

Educate patients and families on home care procedures, treatment plans, and self-care

Maintain accurate and timely clinical documentation in accordance with Medicare and agency standards

Participate in case conferences and team meetings as needed
Qualifications:

Active Florida RN license (required)

Minimum 1 year of Home Health experience (Medicare-certified agency)

Current BLS certification

Knowledge of OASIS documentation (required)

Strong organizational and communication skills

Reliable transportation, valid driver’s license, and current auto insurance
