Description
We are seeking a licensed Physical Therapist Assistant (PTA) to provide skilled therapy services to home health patients under the direction and supervision of a licensed Physical Therapist (PT). The PTA will assist patients in improving mobility, managing pain, and recovering function following illness, surgery, or injury—all in their home environment.
Responsibilities:

Provide physical therapy treatments and interventions as outlined in the Plan of Care developed by the supervising Physical Therapist.

Collaborate with the PT, RN case managers, and other clinicians to ensure continuity and effectiveness of patient care.

Monitor and document patient progress, reporting changes in condition or response to therapy to the PT and clinical team.

Educate patients and caregivers on exercises and safety practices to enhance mobility and prevent injury.

Complete accurate and timely clinical documentation in compliance with Medicare, state regulations, and agency policies.

Travel to patient homes within the assigned territory.
Qualifications

Graduate of an accredited Physical Therapist Assistant program.

Current PTA license in the state of Florida.

Minimum of 1 year of experience as a PTA preferred, home health experience a plus.

Current CPR certification.

Reliable transportation, valid driver’s license, and auto insurance.

Ability to work independently and communicate effectively with patients, caregivers, and team members.

Experience with electronic documentation systems (e.g., Wellsky) preferred.
