Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA)

PRN – Home Health

Overview:
We are seeking a dedicated Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) to join our home health team on a PRN basis. In this role, you will help patients safely regain independence, improve functional abilities, and enhance their quality of life within the comfort of their home environment.

This position offers flexible scheduling and the opportunity to provide meaningful, one-on-one care while being supported by a collaborative clinical team.

Key Responsibilities (OT):

• Perform comprehensive occupational therapy evaluations for home health patients.

• Develop individualized treatment plans to improve ADLs, functional mobility, safety, and overall independence.

• Implement therapeutic interventions and adjust plans of care based on patient progress.

• Provide patient and caregiver education on safe home practices, adaptive equipment, and home exercise programs.

• Collaborate with nursing, PT, SLP and other clinical team members to coordinate care.

• Complete accurate, timely documentation according to Medicare and agency standards.

Key Responsibilities (COTA):

• Deliver occupational therapy treatments under the supervision and direction of a licensed OT.

• Assist patients with therapeutic activities aimed at improving ADLs, fine motor coordination, strength, and functional mobility.

• Document all visits and communicate patient progress or concerns to the supervising OT.

• Reinforce patient and caregiver education according to the plan of care.

• Collaborate with the care team to ensure continuity and quality of care.

Qualifications

• Active Florida OT or COTA license.

• Home health experience 

• Strong clinical and problem-solving skills.

• Ability to work independently in the field.

• Reliable transportation, valid driver’s license, and active auto insurance.

• Excellent communication and electronic documentation skills.
