Duties
- Assist residents or clients with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor patients’ vital signs, observe health status, and report changes to management/family promptly
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption as needed
- Maintain cleanliness by performing light housekeeping tasks, including cleaning patient areas and ensuring a hygienic environment
- Provide companionship and emotional support, fostering a positive and respectful atmosphere and quality of life for clients
- Transportation in client vehicles only.
Skills
- Proven experience working in homes, assisted living facilities, nursing homes, or home health environments
- Strong knowledge of patient observation, monitoring vital signs, and recognizing signs of distress or deterioration
- Familiarity with mild neurodevelopmental challenges
- Ability to assist with ADLs and provide personal care assistance with compassion and professionalism
- Knowledge of HIPAA regulations to ensure confidentiality and privacy of patient information
- Skilled in using patient transfer methods and understanding patient transfer safety protocols
- Excellent communication skills for effective client interaction and teamwork within multidisciplinary care teams
- Ability to handle heavy lifting under 50 lbs safely and perform manual handling tasks as required
- Experience working with diverse populations/seniors
- Strong organizational skills for maintaining accurate documentation related to patient care and medication administration
- Computer or phone- based clock in, scheduling. reporting
