Role Description
This is a full-time on-call role for a Care Coordinator based in San Antonio, TX. The Care Coordinator will be responsible for overseeing care plans, conducting case management, coordinating with healthcare providers, and ensuring consistent communication with clients and their families. Daily tasks also include customer service duties and home care management to enhance the quality of life for seniors under our care.
Qualifications
- Care Coordination and Case Management skills
- Communication skills both with clients and healthcare providers
- Home Care management experience
- Customer Service skills
- Compassionate and empathetic personality
- Ability to work independently and make sound decisions
- Experience in senior care or healthcare setting is a plus
