ACTIVITIES COORDINATOR/SOCIAL SERVICES DESIGNEE

The Activities Coordinator shall spend a sufficient number of hours per week in the facility to plan, carry out, and document the activity program. The total number of hours shall be sufficient to meet the needs and interests of the patients.

The Social Services Designee (SSD) is responsible for supporting the SNF/LTC Director of Nursing in compliance with all federal, state, and local laws and regulations pertaining to the provision of clinical social services.

 

Essential Functions

Activities Coordinator

  • Develop, implement, and supervise the activity program in accordance with OBRA and Title 22.

  • Develop, implement, and supervise the budget and the fund raising for the residents' activity fund.

  • Monitor and maintain, in conjunction with the DON, the residents' individual accounts up to a maximum of $50.00 for reach resident.

  • Must attend on a weekly basis each resident's quarterly care conference and document quarterly progress notes.

  • Participate in developing Care Planning for each resident and update as needed.

  • Assist residents to medical appointments as needed.

  • Attend monthly Seneca Healthcare Auxiliary meetings and report activities to the membership.

  • Develop appropriate contacts with community agencies and organizations.

  • Conduct or participate in in-service training for the staff of the facility, and where appropriate and permissible, recruit, train, and supervise volunteers to assist with or augment the regular program.

  • Obtain equipment and supplies for the program.

  • Post the current activity calendar conspicuously for the information of patients and staff and provide the DON with a current schedule of planned activities.

  • Maintain a current record of the type and frequency of activities provided and the names of patients participating in each activity.

  • Coordinate the activities schedule with other patient services.

  • Develop and maintain an appropriate schedule of activities for patients that are unable to leave their rooms.

  • Follow all general and departmental safety, health and security policies and procedures. Utilize all safe work practices for department.

  • Attend and participate in in-service educational classes and on-the-job training programs as required or directed.

  • Maintain CONFIDENTIALITY of all pertinent patient/resident care information to assure patients/residents' rights are protected.

  • Complete annual employment health requirements in a timely manner.

  • Assist with CNA duties while on outings and when needed in the facility.

  • Other related duties as may become necessary or as directed by Immediate Supervisor (LVN Charge Nurse) or the DON.

 

Social Services Designee

  • Follow all Title 22 and OBRA regulations

  • Schedule appointments for residents

  • Must attend on a weekly basis each resident’s quarterly care conference and document quarterly progress notes

  • Perform initial resident assessments

  • Perform weekly resident visits and document appropriately

  • Assist with insurance, billing, and any other legal issues that arise

  • Work with ombudsmen regarding grievances and other issues

  • Assist Facility personnel in the completion of social history and assessment by in-servicing staff.

  • Provide direct counseling to the patients of the facility (and such patients’’ families) when problem areas are identified, and counseling is required. Set up counseling sessions on a mutually agreeable scheduled basis, and/or at other times as required or requested by SNF/LTC Director of Nursing or CNO/CEO.

  • Maintain a current active file of available community resources for social needs and make such file available.

  • Periodically advise and educate the professional staff of the Facility on all social service matters.

  • Review and supervise documentation of the social case section of the medical-social care plan of the facility on a regularly scheduled basis.

  • Shall provide an accurate report to the Facility’s SNF/LTC Director of Nursing as required by the state and/or federal regulations.

  • Other related duties as may become necessary or as directed by Immediate Supervisor (LVN Charge Nurse) or the DON.

  • Follows safe work practices, takes an active interest in preventing injury or illness and promoting a safe and healthful environment for self and others, and complies with Hospital and governmental safety regulations.

  • Must function independently, have personal integrity, have flexibility and the ability to work effectively with other personnel, clients, and support agencies

  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours and ability to lift up to 20 lbs.

Qualifications:

Qualifications

  • Certified Nurse Assistant certification.
  • Current BLS certification.
  • Social Services Designee Certificate and at least 3 years work experience.
  • Must have two (2) years’ experience in a social or recreational program within the past five (5) years; one (1) year of which was full time in a patient activities program in a health care setting, or:
  • Be an occupational therapist, art therapist, music therapist, dance therapist, recreation therapist, or occupational therapy assistant, or:
  • Have completed at least 36 hours training in a course designed specifically for this position and approved by the Department of Health Services and shall receive regular consultation from an occupational therapist, occupational therapy assistant, or recreation therapist who has at least one (1) year experience in a health care setting.
