What are the responsibilities and job description for the Occupational Therapist, Home Health, Full Time position at Self Regional?
Provides Occupational Therapy (OT) services to patients by means of assessment, planning and implementation of treatment. Remains proficient in implementation of clinical procedures such as therapeutic activities, modalities, adaptations, and patient and family education and assessment ofdurable medical equipment (DME) needs and uses these skill in the home setting. Maintains personal professional growth through continuing education and independent study and share information and training with team members. Fosters open communication and rapport with other disciplines, physicians, all hospital disciplines and outside agencies as needed to ensure patient needs are met. Completes all documentation per Home Health policies. Performs all other duties as assigned by Home Health Services (HHS) Managers and Directors.
Must be a graduate of an Occupational Therapy program. Must hold a SC License in OT. NBCOT Certification required. Home Health experience preferred. BLS certified.
