Duties
- Assist individuals in developing and implementing their self-directed plans, ensuring that their needs and preferences are prioritized.
- Facilitate communication between individuals and service providers to ensure that all parties are informed and engaged.
- Provide training and resources to help individuals understand their rights, responsibilities, and available options within the self-direction framework.
- Monitor progress towards goals, adjusting plans as necessary to ensure continued alignment with individual aspirations.
- Advocate for individuals’ needs within the community and assist them in accessing necessary services and supports.
- Maintain accurate records of interactions, progress, and any changes in individual circumstances.
Experience
- Previous experience in social work, counseling, or a related field is preferred but not required.
- Strong interpersonal skills with the ability to build trust and rapport with diverse populations.
- Knowledge of self-direction principles and practices is a plus.
- Excellent organizational skills with attention to detail to manage multiple cases effectively.
- Ability to work independently while also being a collaborative team member.
