Job Summary

Select Home Health and Hospice is seeking a skilled and compassionate Admissions Registered Nurse to complete patient admission visits across the Indianapolis area. This floating position supports both home health and hospice services, ensuring patients receive thorough initial assessments, individualized plans of care and a smooth transition into services.

Duties/Responsibilities:

• Complete patient admission visits and initial assessments for home health and hospice patients.

• Perform patient assessments and ongoing evaluations to monitor patient needs and changes in condition.

• Develop and implement individualized plans of care in collaboration with physicians and the clinical team.

• Complete required clinical documentation, including OASIS assessments for eligible home health patients.

• Provide in-home nursing care and additional patient visits as needed based on agency needs.

• Educate patients and caregivers on medications, diagnoses, treatment plans and in-home care.

• Coordinate patient services and communicate changes in patient condition to physicians and the appropriate care team.

• Float throughout the Indianapolis area to provide admission coverage and additional nursing support as needed.

• Maintain compliance with applicable regulations, clinical standards and agency policies.

Physical Requirements

• Ability to sit, stand, and drive for extended periods while providing in-home care.
• Ability to assist and lift patients, as well as move equipment or supplies, with or without reasonable accommodation.
• Regular use of a tablet for documentation, communication, and training.

• Graduate of an accredited school of professional nursing (required).
• Current Registered Nurse license for the practicing state of Indiana (required).
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience (preferred).
• Valid driver’s license, reliable auto insurance, and personal transportation (required).
• Current up to date TB test.
