The Case Manager serves the Grimes Family Center, a 36-unit recovery housing community for women and children located within the Seeds of Hope Recovery Campus.
The Grimes Family Center is home to Seeds of Change, a free health clinic, and St. Mary's Early Childhood Center, creating a unique environment where housing, healthcare, childcare, recovery support, and case management are integrated to help families achieve long-term stability.
The Case Manager provides comprehensive case management services to residents, helping women and families achieve stability through coordinated housing support and access to recovery services, healthcare, childcare, employment, education, and other community resources. Working collaboratively with Volunteers of America (VOA), Seeds of Change Health Clinic, St. Mary's Early Childhood Center, and community partners, the Case Manager develops individualized service plans, coordinates care, advocates for residents, and supports them in achieving their goals and long-term independence.
The Case Manager embraces a person-centered, trauma-informed, and recovery-oriented approach while promoting hope, accountability, dignity, and self-sufficiency.
Pay: $52,000.00 - $57,000.00 per year
