What are the responsibilities and job description for the Case Manager position at SEBCO?
Bronx Men’s Homeless Shelter seeks to hire a Full-Time Case Manager who will assist residents in seeking and obtaining services available throughout their transition from temporary housing to permanent housing.
RESPONSIBILITIES/DUTIES/FUNCTIONS/TASKS
- Establish professional relationships with residents while ensuring their confidentiality.
- Create and maintain residents' charts, monitoring and documenting their progress.
- Conduct an initial intake of residents, entering all accurate information into CARES within 48 hours or as stipulated by DHS/OTDA.
- Update the Independent Living Plan (I.L.P.) on a weekly or biweekly basis, monitoring and logging residents' progress through regular meetings. All progress notes must be entered on the same day the I.L.P. is conducted.
- Complete a Psycho-Social assessment in CARES within the first month of a resident’s stay in the shelter, with updates required yearly unless otherwise requested.
- Schedule appointments for residents with referral organizations and follow up with both clients and referral organizations regarding contact and progress.
- Update clients’ engagement and progress on a weekly basis.
- Act as a resident liaison and advocate with outside organizations on matters such as education, healthcare, housing, and legal issues.
- Assist clients in addressing barriers to obtaining suitable housing.
- Help clients maintain an open and active Public Assistance case.
- Escort clients when necessary.
- Assist clients in obtaining necessary documents, such as NYS ID and birth certificates, for both documented and undocumented residents.
- Support clients in their employment search, including updating resumes, cover letters, and applications.
- Refer clients to outside resources for employment opportunities.
- Attending staff meetings and training sessions as scheduled.
- Perform other duties assigned by the Program Director.
- Education and Experience:
- Bachelor’s degree with 2 years’ work experience preferred.
- Computer literacy.
- Good oral, writing, and analytical and problem‐solving skills.
- Perform other duties as assigned and responsibilities as required.
Shift / Schedule:
7 hours per day, 5 days per week, dependent on operational needs.
Qualifications:
Bachelor’s degree in Social Work or another related field desired
Strong Microsoft Office and Excel, and database entry skills
Bilingual English/Spanish a plus
