Seasons Hospice of Muskogee is seeking a compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to join our team on a PRN basis, completing in-person face-to-face (F2F) visits for hospice patients throughout our service area.

This is an excellent opportunity for an advanced practice provider who values flexibility and autonomy while still wanting to do deeply meaningful work. You'll collaborate with our Medical Director and interdisciplinary hospice team to help ensure our patients receive compassionate, consistent care wherever they call home.

What You'll Do

A primary responsibility of this position is completing required in-person hospice face-to-face (F2F) visits. Visits may take place in patients' homes, facilities, or other care settings throughout our service area.

You will:

• Conduct in-person hospice face-to-face visits and comprehensive patient assessments.
• Evaluate patients' current clinical status and changes in condition.
• Perform physical examinations and clinical evaluations.
• Collaborate with the Medical Director and interdisciplinary team regarding patient needs and continuity of care.
• Develop treatment recommendations and provide clinical guidance within your scope of practice.
• Prescribe medications and treatments when appropriate and within scope.
• Complete timely, thorough, and accurate clinical documentation.
• Provide education and support to patients, families, and caregivers.
• Serve as a clinical resource to members of the hospice care team
• Travel throughout the assigned service area as necessary to meet patient care needs.

Travel & Service Area

Our patients are located throughout Muskogee, Henryetta, Tahlequah, and surrounding communities. Candidates must be willing and able to travel to the necessary locations based on patient needs to ensure continuity of care. Reliable transportation is required, and mileage is reimbursed.

What We're Looking For

We're looking for a provider who is compassionate, dependable, clinically confident, and comfortable working independently in the field while remaining closely connected to the hospice team.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current Oklahoma licensure and appropriate authorization to practice.
• Current RN license for Nurse Practitioner candidates, as applicable.
• Strong clinical assessment and decision-making skills.
• Excellent communication, documentation, and interpersonal skills.
• Reliable transportation and willingness to travel throughout the service area.
• Hospice, palliative care, geriatrics, home health, family practice, or internal medicine experience preferred.
• Hospice or palliative care experience strongly preferred.
