Lead with Vision: You’ll direct the entire Social Services department, ensuring we not only meet federal and state regulations but exceed the NASW Code of Ethics.
Be the Voice for Others: You will serve as the primary advocate for resident rights, ensuring their psychosocial needs are met and their voices are heard within the interdisciplinary team.
Navigate the Journey: From the moment of admission to a successful discharge, you will guide patients and families through transitions, providing clinical support and expert resource coordination.
Build the Dream Team: You will recruit, mentor, and inspire a team of dedicated social workers (or manage the function individually in specialized centers), fostering professional growth and excellence.
Key Responsibilities
- Clinical Excellence: Conduct deep-dive psychosocial assessments, develop interdisciplinary care plans, and provide therapeutic interventions for residents adjusting to long-term care.
- Strategic Administration: Manage departmental budgets, staffing needs, and Quality Improvement initiatives. You’ll be the key Social Services voice in Morning Meetings and Utilization Management.
- Discharge Mastery: Coordinate seamless transitions back to the community by building strong linkages with behavioral health, hospice, and other community-based providers.
- Empowerment through Education: Train nursing center staff on resident rights and abuse prevention while educating families on healthcare decision-making and advanced directives.
- Master’s degree in Social Work from an accredited school of Social Work required
- Must possess any certifications/licensures as required by State of employment to practice
- Additional certification such as Geriatric Case Management, Hospice & Palliative Care,
Substance Abuse preferred.
- At least 3-5 years of supervised Social Service experience in a health care setting working
- Management/administrative/supervisory experience preferred.
