$22.00-$38.00hr
MAIN FUNCTION:
- To be responsible for the direct treatment of an assigned caseload of patients who are addicted to alcohol or any other mind/mood altering substances.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Ensure that all clients are treated with dignity and respect.
- Affirming care for Lesbian, Gay, Bisexual, Transgender and Questioning clients.
- Provide group, individual, and family counseling.
- Complete assessments, admissions, and treatment plans (TPRs).
- Document all services provided.
- Maintain contact with referral sources (EAPs, Criminal Justice).
- Send Referral Progress Reports.
- Participate in weekly case conferences, supervision, and staff meetings.
- Ensure compliance with Seafield policies and confidentiality regulations.
- Complete discharge planning, UR, and referrals.
- Perform other duties as assigned.
