Role: The Registered Nurse (RN) provides safe, patient-centered care in a home health setting, utilizing the nursing process to evaluate and coordinate care collaboratively with interdisciplinary teams.

- Deliver nursing care following physician orders and develop individualized care plans with measurable goals.
- Monitor patient progress, communicate changes to the care team, and educate patients and caregivers on treatments and medications.
- Coordinate services with team members such as PT, OT, ST, and MSW.
- Perform treatments in compliance with standards, assess home safety, and report concerns including neglect or abuse.
- Complete accurate documentation per regulatory guidelines and participate in on-call rotations, weekends, and holidays as needed.
- Support quality improvement and patient satisfaction initiatives.
- Graduate of an accredited nursing program with current RN licensure.
- Valid driver’s license and CPR certification.
- Strong organizational skills, adaptability, and ability to work independently.
- Comfortable with digital documentation and technology.
- Preferred: 1 year of relevant nursing experience, especially in home health.
