Responsibilities
- Participate in daily team meetings.
- Assist with medication administration, including IMs and oral meds.
- Maintain medication schedules and coordinate with prescribers and pharmacies.
- Transcribe medication orders and update records.
- Log medication deliveries and complete toxicology forms.
- Support nursing assessments and document progress notes.
- Conduct weekly Healthy Living group sessions with consumers.
- Perform additional duties as directed by the Team Leader, aligned with program goals.
- Licensed LPN in New York with an associate's, bachelor's, or master's degree.
- Experience working with mentally ill or chemically addicted populations.
