Key Duties
- Conduct initial and ongoing social work evaluations.
- Communicate findings and care needs to healthcare teams.
- Implement and modify care plans based on patient progress and needs.
- Counsel patients and families, providing education and support.
- Document case activities accurately and timely.
- Master’s or Doctoral Degree in Social Work from an accredited program.
- Current State License as a Social Worker.
- Minimum of 1 year healthcare social work experience.
- Excellent communication skills.
- Ability to travel up to 50% and perform physical tasks including patient transfers.
- Valid driver’s license and reliable transportation.
