We are seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover from illness or injury at home.
Key Responsibilities Include
- Conduct initial and ongoing assessments of mobility, coordination, strength, and function.
- Administer, evaluate, and interpret tests to set functional goals and outcomes.
- Develop and document personalized care plans, including progress and discharge plans.
- Deliver therapy services in accordance with physician orders and modify treatments as needed.
- Collaborate with healthcare teams, patients, and caregivers, providing education and discharge planning.
- Document accurately to ensure compliance and prevent denials, with minimal supervision.
- Supervise other personnel such as PT Assistants and Home Health Aides where applicable.
- Complete OASIS assessments within scope of practice and adhere to policies, regulations, and laws.
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license as a Physical Therapist.
- Valid CPR certification.
