Key Responsibilities
- Providing patient education and support to promote health and wellness.
- Coordinating care across services to ensure comprehensive patient management.
- Identifying and overcoming barriers related to socioeconomic factors and health literacy.
- Assisting patients in navigating healthcare systems and resources.
- Monitoring and managing individual patient cases to optimize outcomes.
- Associates Degree in Nursing and minimum 2 years of nursing experience.
- Current BLS certification from AHA or ARC.
- Active Registered Nurse license (Georgia) or multi-state compact license.
- Preferred: Bachelor's Degree in Nursing, over 1 year of case management experience, and national certification in case management.
