The Annual Wellness Visit (AWV) is a Medicare (primarily age 65 ) designed program that encourages beneficiaries to visit their primary care physician at least once per year to assess their health. To adequately assess patient health, the appointment is considerably longer than a routine office visit. A history of medications, comprehensive list of other physicians the patient may utilize, as well as a variety of screenings (ambulatory, alcohol, depression, etc.) are conducted with the patient. It is not unusual for these appointments to be 30-45 minutes compared to the normal 15-20 minute time allotments used for a majority of appointments.

The Clinical Care Coordinator works as an extension of the physician to conduct a phone interview and complete a majority of the required AWV information prior to the actual office visit. The Clinical Care Coordinator conducts the form completion for the patient and the physician’s office to improve patient flow within the office and deliver an enhanced experience to the patient. There are multiple parts to the process:
- Identification of Patients with an AWV Appointment
- Abstraction and transcription of Medical History of Patient
- Communication with Patient Prior to the Appointment
- Manage inbound and outbound patient interactions via telephone (primarily) to complete the Medicare Annual Wellness Visit forms.
- Utilize customer systems (scheduling platform, reports, etc.) to identify upcoming AWV appointments.
- Capture data in clean, concise format in accordance with customer specifications.
- Assist with education of upcoming appointment times and requirements for visit.
- Follow customer protocols to assess and complete documentation required for services.
- Observe privacy standards and conform with patient confidentiality (HIPAA) requirements.
- Other duties as assigned by program manager/supervisor.
- Licensed Practical Nurse (LPN) license required - Must be licensed in State of Georgia or have / willing to obtain compact multi-state license
- Minimum 3 years of experience working in a doctor’s office or hospital setting
- Minimum 3 years of experience working with patients
- You have a high school diploma (required); College or some college preferred
- You have exceptional customer service and phone etiquette
- You show attention to detail, including grammar, spelling, and punctuation
- You have ability to maintain effective and organized communication
- Self-starter personality and ability to manage time wisely while working as a Team player
- Healthcare field and medical specialty
- Medical terminology
- Knowledge of EHRs (if applicable)
- Typing speed of at least 55 wpm
- Multilingual English/Spanish (preferred but not required)
- Remote Role (SCP-Home Based)
- Must reside in Southeast United States (preference is Georgia & Louisiana) and able to work Eastern time zone
