by abuse through professional assessment, counseling, and education in a
compassionate and collaborative approach.
Job Classification: Full-Time Employee/Not Exempt
Essential Functions:
- Family & Caregiver Engagement
- Gather relevant information from caregivers and MDT partners to assess needs and identify appropriate services.
- Provide clear communication and supportive guidance to caregivers throughout the child’s involvement with the CAC.
- Screening & Intervention
- Complete trauma and suicide risk screenings for children and adolescents.
- Follow established response protocols, including completion of safety planning and/or necessary interventions.
- Document screenings, safety plans, and follow-up interventions accurately and in a timely manner.
- Mental Health Coordination
- Attend mental health case briefings to staff all referrals for services and collaborate with mental health providers.
- Communicate identified mental health needs and service recommendations to caregivers.
- Provide psychoeducation to caregivers on trauma-related symptoms and ways to best support their child.
- Ongoing Case Support
- Conduct structured check-ins with families on a routine basis to monitor progress and ensure engagement in recommended services.
- Follow up with families on barriers to attendance or service participation, helping to identify solutions and remove obstacles.
- Maintain collaborative relationships with MDT partners to ensure continuity of care and coordinated service delivery.
Qualifications:
- Licensed Master’s degree in social work or an individual currently enrolled in a social work master’s degree program and actively progressing towards licensure.
- Two years work experience working with children and families impacted by trauma, abuse, or mental health needs.
- Bilingual preferred (Spanish/English)
- Strong interpersonal, communication, and organizational skills.
- Ability to work collaboratively within a multidisciplinary team environment.
- Effectively work with others in a team environment and maintain composure in high stress situations.
- Must have valid driver’s license and proof of liability insurance.
- Must pass criminal and civil background checks.
- Able to maintain confidentiality and present in a positive, professional manner at all times.
- Regular and reliable attendance.
