Job Overview
We are seeking a dedicated and compassionate Qualified Developmental Disability Professional (QDDP) to join our dynamic team. Duties

  • Conduct comprehensive assessments of individuals’ needs, strengths, and preferences to inform personalized care plans.
  • Develop, implement, and monitor individualized treatment and support plans aligned with each person’s goals.
  • Collaborate with multidisciplinary teams including healthcare providers, social workers, families, and community agencies to coordinate services.
  • Provide direct support and advocacy to promote community integration, daily living skills, and behavioral health stability.
  • Ensure compliance with all relevant regulations such as HIPAA (Health Insurance Portability and Accountability Act) and maintain accurate documentation in medical records.
  • Participate in discharge planning, intake procedures, utilization management, and diagnostic evaluations to optimize care delivery.
  • Engage in case management activities including patient assessment, post-acute care planning, outpatient services coordination, and inpatient support when necessary.
  • Maintain up-to-date knowledge of medical terminology, physiology, addiction counseling techniques, mental health counseling strategies, and behavioral health interventions.
  • Support patients through crisis intervention, emergency response coordination at Level I Trauma Centers or Level II trauma centers as needed.
  • Assist in managing complex cases involving PICU (Pediatric Intensive Care Unit), ICU (Intensive Care Unit), or hospital medicine environments to ensure comprehensive patient care.

Qualifications

  • Bachelor’s degree in social work, psychology, nursing, or a related field; Master’s degree preferred.
  • Valid certification as a Qualified Developmental Disability Professional (QDDP).
  • Experience working in hospital settings such as Level I Trauma Centers or Level II Trauma Centers is highly desirable.
  • Demonstrated knowledge of acute care environments including ICU experience and hospital medicine practices.
  • Familiarity with outpatient clinics, post-acute care services, inpatient units, and managed care systems.
  • Strong understanding of medical terminology, patient assessment techniques, discharge planning processes, and medical records management.
  • Experience with behavioral health interventions including addiction counseling and mental health support.
  • Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams.
  • Ability to handle complex cases involving pediatrics or post-acute care settings confidently and compassionately.
