Our company is looking to hire an admission nurse to work with our Home Health Care clients.This role is ideal for nurses who value autonomy,flexible scheduling and the opportunity to deliver one on one care that truly makes a difference.
Required Certifications and Licensees:
- Licensed to practice as a registered nurse in Massachusetts.
- Must possess and maintain valid CPR certification while employed in a clinical role.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
Responsibilities
- Conduct initial nursing assessments, re-certifications, and reassessments.
- Provide per-visit nursing care such as medication management, vitals, follow-up checks, and wellness visits.
- Supervise caregivers and provide in-field training and support.
- Monitor client progress and report changes in condition to appropriate parties.
- Participate in incident or fall follow-up and update care plans as necessary.
- Communicate effectively with case managers, family members, and care teams.
- Maintain accurate and timely documentation in compliance with state and agency policies.
