Are you a clinician passionate about trauma-informed care and recovery? Join our team to provide high-impact therapy and comprehensive care coordination.
Key Responsibilities:
- Clinical Therapy: Lead individual, group, and family therapy using evidence-based practices (CBT, Seeking Safety, Motivational Interviewing).
- Care Planning: Conduct psychosocial screenings to develop comprehensive care plans, including WRAP and Relapse Prevention.
- Care Coordination: Collaborate with a multidisciplinary team and PCPs to close gaps in care and provide transitional support (CTI) post-hospitalization.
- Field Work: Engage clients through home visits and community-based advocacy to support health outcomes and stable living conditions.
- Documentation: Maintain accurate, timely clinical records within the EHR.
Qualifications:
- Education: Master’s degree in Social Work, Mental Health Counseling, Psychology, or Nursing.
- Experience: 1 years in direct human services, nursing, or care management.
- Licensure: LMSW, LCSW, or LMHC preferred.
- Skills: Proven competency in Suicide Assessment, Trauma-Informed Care, and Psychosocial Rehabilitation.
