Shift: Fulltime Monday through Friday 7am - 6pm (rotating shifts)

Position Summary:
The RN of ADHCP is supervised by the clinical Director of ADHCP. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgement/problem solving, decision making, and initiative.

Key Responsibilities:
- Coordinate services for registrants with services provided by the community or other agency programs including, but not limited to, long term home health care programs, certified home health agencies, social services agencies, clinic, and hospital outpatient departments and services.
- Ensure that duplication of services to individual registrants is avoided.
- Supervise activities of all personnel to ensure that registrants receive assistance in accordance with their needs.
- Secure the services of the registrant's personal physician or alternate when indicated.
- Accept responsibility for providing or arranging inpatient and/or other services for those registrants for whom the program is no longer suitable.
- Arrange transportation services for registrants.
- Interview and assess potential registrants as directed by Director of ADHCP.
- Supervise, direct, and instruct medical needs of ADHCP staff.
- Provide or arrange for all nursing services to registrants.
- Maintain registrant's complete health record.
- Maintain and promote good rapport with all in-house departments.
- Maintain good public relations with outside organizations and facilities, always representing the program in a professional manner.
- Acts as ADHCP Supervisor in absence of Director/Clinical Director. Attend meetings as required.
Qualifications:
Qualifications:
- Education and Training – Currently licensed and registered to practice as a Registered Professional Nurse in New York State. New graduate nurse may be considered. Current cardiopulmonary resuscitation (CPR) certification issued by either the American Heart Association or the American Red Cross. Current driver's license.
- Experience - Minimum of one year nursing experience in a long term care setting is preferred.
- Character - The individual must possess moral character, patience and compassion for the elderly.
