Physical Therapy Assistant (Home Care)

Shift: Full Time Monday through Friday during standard business hours

Position Summary:

The Physical Therapist Assistant a licensed health care professional who provides treatment according to a plan developed by and under the supervision of a licensed Physical Therapist and is responsible for providing service to Schofield Certified Home Care (SCHC) patients in their homes who have mobility problems.

Key Responsibilities:

The Physical Therapy Assistant is supervised by a Licensed Physical Therapist.  The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgment/problem solving, decision making, and initiative.  The responsibilities include, but are not limited to:

• Perform Physical Therapy treatments in accordance with NYS guidelines for practice.
• Conduct individual therapy sessions in the patient's home.
• Prepare progress, recertification, and discharge reports when appropriate.
• Assist in coordination of Physical Therapy program with other health care providers.
• Counsel individual patient/caregiver in relation to patient's specific problems and needs.
• Participate in patient-centered conferences when appropriate.
• Drive a car, possess a valid driver’s license and have a car available for use.
• Follow rules and regulations of the NYS and Federal Health Code in regard to the program.  Interpret and follow Medicare and Medicaid guidelines.

Working Environment:

The Physical Therapist is subject to inside and outside environmental conditions as well as conditions found in patient's homes.  The normal shift is 7 ½ hours between the hours of 7 a.m. and 5 p.m., 5 days per week.  Overtime hours may be encountered.

Job Qualifications:

• Education and Training - Graduate of a school approved by the American Physical Therapy Association with an associate degree.  Must be registered and licensed by the New York State Education Department. 
• Experience - Two years experience preferably with exposure to acute care, rehabilitation or homecare.
• Character – The individual must possess good moral character, patience and compassion for the chronically ill and disabled.

Salary : $23 - $27
