MDS coordinators have a wide range of responsibilities, which can include:
- Providing support to patients and families during treatment by answering questions about medication, disease progression, and other concerns
- Coordinating patient care across disciplines by working with physicians, nurses, social workers, occupational therapists, pharmacists, dieticians, and other healthcare professionals
- Creating treatment plans based on each patient’s medical history and preferences to ensure the best possible outcomes
- Coordinating patient care across multiple facilities to ensure that each patient receives the best possible care
- Reviewing patients’ medical records to identify any issues that need to be addressed by the doctor or other members of the care team
- Managing patient care plans, including setting up appointments with specialty care providers and ordering lab tests
- Enforcing physician orders regarding medication and treatment plans
- Communicating with patients and their families regarding treatment plans and progress
- Managing patient records by creating files, maintaining confidentiality, and ensuring that information is stored safely
