Are you detail-oriented, organized, and passionate about ensuring residents receive the best possible care? Our long-term care community in Beech Grove, IN is seeking an experienced and motivated MDS Coordinator to join our clinical leadership team.
As the MDS Coordinator, you’ll play a vital role in managing the Resident Assessment Instrument (RAI) process, ensuring accurate and timely completion of MDS assessments, and driving quality care outcomes for our residents.
Key Responsibilities:
- Coordinate and complete MDS assessments in compliance with federal and state regulations
- Work collaboratively with interdisciplinary teams to develop individualized care plans
- Ensure accurate documentation to support care delivery and reimbursement processes
- Monitor resident progress and participate in quality improvement initiatives
- Provide guidance and support to nursing staff regarding MDS requirements
Qualifications:
- Current RN or LPN license in the state of Indiana (required)
- Previous MDS/RAI experience strongly preferred
- Strong knowledge of long-term care regulations and clinical documentation
- Excellent organizational, analytical, and communication skills
- Ability to work effectively with interdisciplinary teams
