Job Overview
We're looking for a Clinical Pharmacist who is passionate about delivering evidence-based, patient-centered care while serving as a trusted medication expert across interdisciplinary teams. What You'll Do
- Partner with physicians, nurses, and healthcare teams to optimize medication therapy.
- Perform medication reviews, clinical interventions, and therapeutic recommendations.
- Identify and prevent medication-related problems while improving patient safety.
- Participate in multidisciplinary rounds and provide expert pharmacotherapy guidance.
- Support medication reconciliation and patient education at discharge.
Lead Pharmacy Excellence
- Drive clinical pharmacy programs and medication stewardship initiatives.
- Evaluate and improve medication-use processes through quality improvement projects.
- Monitor program outcomes and implement evidence-based best practices.
- Help advance pharmacy services through innovation and continuous improvement.
Mentor & Collaborate
- Precept pharmacy students and support professional development.
- Collaborate with clinical teams to enhance patient care and operational excellence.
- Serve as a trusted pharmacy resource across the organization.
Required Qualifications
- Graduate of an accredited College of Pharmacy.
- Active Iowa Registered Pharmacist license.
- 2-4 years of hospital-based clinical pharmacy experience.
- Strong clinical judgment and evidence-based decision-making skills.
- Excellent communication and collaboration skills.
Preferred Qualifications
- PharmD degree.
- ASHP-accredited PGY-1 Residency.
- Board Certified Pharmacotherapy Specialist (BCPS).
- Experience leading clinical initiatives or quality improvement projects.
