Position Title: Case ManagerLocation: PHP/IOP Treatment CenterSchedule: Part-time, Flexible ScheduleCompensation: $23–$26 per hour (depending on experience)Position Summary:The Case Manager is responsible for providing individualized support and coordination of care for clients undergoing treatment for substance use and co-occurring mental health disorders. This role ensures that each client's treatment experience is well-organized, goal-oriented, and leads to a successful transition to the next level of care. Key Responsibilities:Conduct biopsychosocial assessments and gather clinical and demographic information upon admissionDevelop and update individualized treatment plans in collaboration with the clinical teamCoordinate referrals and discharge planning, including outpatient treatment, sober living, transportation, and legal or employment-related needsFacilitate case management sessions with clients to assess progress and address barriers to recoveryCommunicate and collaborate with family members, legal representatives, and outside providers (with proper consent)Document all services, progress notes, and treatment plan updates in the electronic medical record (EMR) in a timely and accurate mannerAttend and participate in clinical team meetings, case reviews, and shift handoffsMonitor client compliance with program rules and provide feedback or interventions as neededProvide crisis support and assist with safety planning as appropriateMaintain up-to-date knowledge of community resources and recovery support services Qualifications:Minimum 1 year of experience in a behavioral health, SUD, or residential treatment settingStrong understanding of the recovery process, case management principles, and community resource coordinationProficiency with EMR systems (Kipu experience preferred)Excellent communication, organizational, and problem-solving skillsAbility to work independently while also collaborating with multidisciplinary teamsCPR/First Aid certification (or willingness to obtain upon hire) Preferred Qualities:Familiarity with ASAM criteria and discharge planning best practicesExperience working with dual-diagnosis populations
