Your Role & Responsibilities:
- Organization and maintenance of the facility’s medical records.
- Complete documentation collection of medical records request.
- Prioritize audits activities that focus on high-risk, high-volume, or problem-prone areas as identified in the facility assessment that reflects the specific units, programs, departments and unique population the facility serves.
- Complete daily, weekly, monthly and quarterly audits per facility protocol.
- Complete MDS assessments and care plans for assigned residents.
- Ensure accurate and timely completion of MDS, RAPs, and triggers according to regulatory guidelines.
- Monitor completion of MDS documentation by interdisciplinary team members within required timeframes.

Qualifications:
- Active KY Nurse License
- Knowledge of MDS, Care Plan Process, PPS, and RUGs preferred
- 2-4 years of recordkeeping experience in long-term healthcare preferred.
- Familiarity with resident records and recordkeeping protocol
- Ability to work both independently and collaboratively
