Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long Term Care team. The ideal candidate will possess a strong foundation in patient care, with experience in geriatrics, memory care, and chronic disease management. This role offers an opportunity to work in a compassionate environment, providing essential nursing services to residents in a long-term care setting. The LPN will be responsible for delivering high-quality care, ensuring patient safety, and supporting the overall well-being of our residents.
Duties
- Administer medications and injections accurately following physician orders and protocols
- Conduct comprehensive patient assessments including vital signs, physical examinations, and health histories
- Manage IV infusions and catheter care with sterile technique
- Monitor patients' conditions and report changes promptly to the healthcare team
- Document all patient interactions and nursing interventions using EMR/EHR systems such as Epic, Cerner, or eClinicalWorks
- Provide patient education on medication management, health coaching, and discharge planning
- Maintain infection control standards and adhere to HIPAA regulations to ensure patient confidentiality
- Support residents with dementia or Alzheimer's in memory care units by implementing specialized behavioral health strategies
Experience
- Minimum 1 year of experience required.
- Hawaii LPN license
- BLS
