ECM Trainer – Care Coordinators & CHWs

Schedule
Full-time position. Scheduling varies based on operational needs. We can discuss maintaining the employee’s current schedule or the possibility of transitioning to a Monday through Friday schedule.
About the Position
The ECM Trainer is responsible for developing, coordinating, and delivering training programs for ECM Care Coordinators and Community Health Workers. This role supports both new hire onboarding and ongoing education to ensure staff are equipped with the knowledge, tools, and workflows necessary to provide high-quality, patient-centered care.
The ECM Trainer collaborates closely with leadership and other team members to identify workflow gaps, reinforce best practices, support regulatory compliance, and improve operational efficiency across the Population Health department.
Key Responsibilities
- Facilitate onboarding and training for newly hired ECM Care Coordinators and CHWs
- Assist in development and maintenance of training materials, workflow guides, competency checklists, and educational resources in collaboration with other ECM trainers
- Provide ongoing coaching and support related to ECM workflows, documentation standards, patient engagement, and field operations
- Conduct refresher trainings and workflow updates when new processes, payer requirements, or operational changes are implemented
- Monitor staff competency and provide individualized support to improve performance and confidence
- Partner with leadership to identify workflow barriers, training opportunities, and operational improvement initiatives
- Support education around social determinants of health, care coordination best practices, patient outreach, and community resource navigation
- Train staff on compliance standards, privacy regulations, documentation expectations, and organizational policies
- Assist with quality assurance initiatives, chart audits, and documentation review to ensure program compliance
- Collaborate with interdisciplinary teams to ensure alignment between clinical operations and field-based care coordination services
- Support field shadowing, mentorship, and hands-on training opportunities for ECM staff
- Promote a positive, collaborative, and supportive learning environment that encourages staff growth and retention
Our Ideal Candidate Will Have the Following Qualifications
- Minimum of 1 year of experience in care coordination, case management, population health, ECM, or community-based healthcare programs
- Prior experience training, mentoring, onboarding, or leading healthcare staff strongly preferred
- Strong understanding of Enhanced Care Management (ECM), UTC workflows and social determinants of health
- Experience working with vulnerable, high-risk, or chronically ill populations preferred
- Excellent communication, presentation, and interpersonal skills
- Ability to effectively educate and support diverse learning styles
- Strong organizational skills with the ability to manage multiple projects and priorities
- Proficiency in electronic health records (EHR), Microsoft Office, and healthcare documentation systems
