Care Manager
Job Summary:
The Care Manager serves as the single point of contact for members, coordinating all care needs across healthcare, social, and community services. This community-based role involves conducting face-to-face assessments, developing individualized care plans, and facilitating care transitions for members with complex health, behavioral, and long-term support needs. The position is critical for ensuring members receive safe, cost-effective, and well-coordinated care in the least restrictive environment.
Key Responsibilities:
- Travel to members’ homes, nursing facilities, and community settings to complete in-person assessments.
- Develop, implement, and evaluate care plans tailored to members’ specific needs.
- Coordinate services across physical, behavioral, and long-term care providers to ensure continuity and integration of care.
- Facilitate transitions between care settings such as hospital-to-home or facility-to-community.
- Educate members and caregivers on health needs, benefits, and available resources.
- Collaborate with care teams, discharge planners, and internal staff to optimize member outcomes.
- Document all services and interventions in the electronic health record following compliance and privacy regulations.
- Monitor effectiveness of care plans and make adjustments as needed.
- Ensure timely provision of approved support services and resolve obstacles to optimal care.
Minimum Education & Experience Requirements:
- Bachelor’s degree in Social Work, Health, Human, or Education Services and 3 years of relevant experience, OR
- Master’s degree in related field and 1 year of relevant experience, OR
- Registered Nurse or Licensed Practical Nurse with 2 years of relevant experience, OR
- High school diploma or equivalent with 3 years of case management experience in aged, disabled, or behavioral health populations.
Special Requirements:
- Reliable transportation and willingness to conduct face-to-face visits starting week 1.
- Must live within 30 miles of assigned members’ homes.
Knowledge, Skills, and Abilities:
- Proficiency with Microsoft Office (Word, Excel, Outlook) and electronic documentation systems.
- Strong organizational, problem-solving, and communication skills.
- Ability to manage multiple cases, meet deadlines, and coordinate complex care plans.
- Cultural competency in addressing diverse populations.
- Flexible schedule to meet member needs, including travel within assigned territory.
Additional Desired Characteristics:
- Certified Case Manager (CCM) or licensed social work credentials (LBSW, LMSW, LCSW).
- Experience with behavioral health, HIV/AIDS, developmental disabilities, or geriatric populations.
- Managed care or Medicare/Medicaid experience.
- Experience in home clinical or case management settings.
