🎯 About the Role
We’re looking for a dedicated Population Health Specialist to join our team and help us continue delivering the level of care our patients and families deserve. In this role, the population health specialist will collaborate with SHMG Operational Leadership to support the execution of care delivery strategies through analysis and assessment of data intended to enhance the overall health of the SHMG patient population. The specialist will be responsible for abstracting and collecting both clinical and other pertinent data from the various insurance companies, electronic medical records, as well as quality metric systems in an effort to identify and prioritize opportunities to close gaps in care and enhance the Medical Group's ability to meet incentive and quality outcome targets through enhanced patient outcomes.
🔍 What You’ll Do
Extract and analyze patient care data to identify missed opportunities for physician incentive programs and value-based payment and performance activities.
Collaborate with insurance companies to prioritize gaps in care and work with insurers to enhance data that is being reported to the organization.
Collaborate with nursing and informatics to ensure that appropriate systems and processes are in place for care capture and reporting.
Develop systems and processes to capture "location of care" for key patients and develop a communication tree to care managers for further outreach.
Develop a robust reporting process to enhance existing quality metric reports. Inclusive of this reporting is a) retrieval to reporting; b) trending and prioritizing; c) tracking incentives and earnings; and d) assistance to ensure that the data drives focus as it relates to overall project goals with various SHMG partners.
Work with Finance Team to ensure that areas of focus/priorities around quality metric tracking/trending have greatest financial impact to the organization.
Collaborate to create/devise communication tools that increase and enhance Physician and nursing awareness for missed opportunities.
Support Physician and Nursing in data comprehension and interpretation.

📋 What You Bring
- Degree or Certification in Healthcare Management or Data Analytics and 5 years' professional experience in a physician practice, data collection, data analytics, or EMR required. More than 5 years' professional experience will be considered in lieu of a degree.
- Certified Coder Certificate and/or LPN certification and an RN (AS or BSN), Healthcare Management or Data Analytics are preferred.
- 5 years of experience in care plans; quality improvement; and/or care coordination or equivalent training/experience working with a patient population that has multiple chronic conditions is desirable.
