Job Summary
The Outpatient Care Coordinator establishes relationships with outpatient services program participants and/or patients to provide effective management of their identified needs (episodic and population-based) through professional interviews to obtain complete and accurate information regarding any applicable outpatient services, while working closely with hospital business and medical services, agencies across the continuum (internal and external) and assistance programs, coordinating any available educational, clinical or support services that address the needs of the clients and/or patients utilizing expertise and involvement in professional activities to serve as a resource person and strong liaison between Sarasota Memorial Health Care System services, the clients and/or patients and other community organizations, agencies and offices.
Required Qualifications
- Require Bachelor’s degree; relevant work experience in a hospital or outpatient health care setting can be substituted on a year-for-year basis for the required degree.
- Require a minimum of one (1) year experience in a hospital or outpatient health care setting. Intern field placement or Master’s degree will be considered in lieu of work experience.

- Prefer demonstrated basic experience using a PC and use of Internet including e-mail/calendar.
- Prefer demonstrated ability to perform public speaking.
- Prefer demonstrated basic understanding of medical terminology.
- Prefer demonstrated ability to establish and maintain effective working relationships.
- Prefer demonstrated ability to work independently without direct supervision.
