Adult Day Health Social Worker

Sarah's Place Adult Day Health Center is dedicated to promoting the health, activity, and social engagement of aging adults within the community. As a non-profit organization established in 2000, we provide structured day programs that stimulate physical, cognitive, and social well-being for adults requiring supportive care.

We are seeking compassionate and skilled Social Workers or Social Services Coordinators to join our team at Sarah's Place Adult Day Health Center. The ideal candidate will have experience working with aging adults.

• Promote the well-being of participants through engagement in adult day health programming.
• Utilize an Electronic Health Record EHR to document participant status, history and complete periodic assessments.
• Assist caregivers to navigate the resources and options available to help them continue to provide assistance to their loved one.
• Collaborate with an interdisciplinary team (Nursing, Activities, Social Work) to coordinate care plans that reflect individuals needs, goals and abilities.
• Conduct thorough assessments to identify the social and emotional needs and develop plans to address the individualized needs of each participant and to ensure participants receive appropriate services at home, in the community or at the adult day health program.

• Bachelor's degree in social work, psychology, or a related field.
• Licensed Social Worker (LSW)
• Proven expertise working with aging adults, individuals with physical, mental or cognitive disabilities.
• Strong skills in communication, assessment and case management are essential.
• Experience working within an adult day health setting, a community-based program, or within the aging network is advantageous.

Job Type: Part-time

Pay: $25.00 - $30.00 per hour
