Position overview
Sapphire Home Health is seeking a motivated and results-driven Home Health Marketer to expand our referral network and grow our patient census. The ideal candidate is an outgoing professional with experience building relationships with physicians, hospitals, skilled nursing facilities (SNFs), assisted living facilities (ALFs), case managers, discharge planners, and other healthcare professionals.
Responsibilities
- Develop and maintain referral relationships with healthcare providers and community partners.
- Promote Sapphire Home Health services to increase patient referrals and census.
- Conduct regular visits to physicians’ offices, hospitals, SNFs, ALFs, and other referral sources.
- Identify new business opportunities and establish strategic partnerships.
- Represent Sapphire Home Health at networking events and community outreach activities.
- Maintain accurate records of marketing activities and referral sources.
- Collaborate with the clinical and administrative teams to ensure excellent customer service.
Pay: $72,000.00 per year
Work Location: Hybrid remote in Ventura, CA 93003
