Responsibilities
- Coordination of scheduling and transportation for medical appointments
- Act as Community Attendant for individuals during local community and health related appointments, ensuring the individual's safety and that information needed for the individual's service plan is exchaged
- Coordination of individualized opportunities for peer and community involvement
- Escorting to and from meals or activities, or bringing meals to the room
- Coordination and access to community resources and services, including but not limited to religious, vocational or educational opportunities or volunteer groups
- Completion of one-on-one supportive tasks to help engage the individual with meaningful opportunities
- Other duties as assigned
