WHO WE ARE: Located in Gresham, OR with easy access to the Max, you'll find Sapphire Liberty Pointe. Offering 35 private apartments to medically complex and behavioral residents in our RCF and 15 boutique like apartment settings in our quaint Memory Care. While fostering a home-like environment, Liberty Pointe provides 24 hour nursing care along with elevated staffing ratio's, higher than what's found at nursing homes. Following our Sapphire developed Radiance program, we bring purpose, joy and mission back into the lives of our residents, while also engaging our staff to promote the highest quality of care. Sapphire supports on going training, tuition reimbursement, competitive wages and generous incentives to all of our employees.

Responsibilities
- Maintain patient confidentiality
- Fosters a supportive, compassionate environment for our residents and their families
- Medication Administration: Prepare and administer medications to residents as prescribed, following all state and federal regulations, facility policies and procedures and maintaining accurate medication records.
- Report changes in residents' conditions to appropriate team members.
- Assistance with Daily Living: Support residents with daily activities such as bathing, dressing, and grooming, ensuring dignity and comfort.
- Care Coordination: Collaborate with nursing and other healthcare professionals to create and maintain individual care plans for each resident.
- Documentation: Accurately document all care, medications, and any observed changes in residents’ conditions in accordance with company policies.
- Emergency Response: Act promptly and calmly in emergencies, following established protocols to ensure resident safety.
- Family Communication: Communicate with residents' families regarding health status, medication changes, and overall care updates, fostering trust and transparency.
- Compliance: Maintain compliance with state and federal regulations, as well as company policies regarding healthcare and safety standards.
- Other duties as assigned
- High school diploma or equivalent required.
- Previous experience in healthcare setting, preferably in assisted living, long-term care or community based care.
- Ability to work independently and as part of a team.
- Patience, empathy, and a genuine desire to help others.
- Good organizational and time-management skills.
- Ability to perform physical tasks such as lifting, bending, and assisting clients with mobility.
- Must be able to stand, walk, and reach for extended periods.
- Ability to pass a background check.
- Experience: Prior experience in a healthcare setting, preferably in assisted living, long-term care, or community-based care.
- Skills: Strong communication skills, attention to detail, and ability to work independently.
- Physical Requirements: Ability to lift and move residents as needed, as well as stand, walk, and sit for extended periods.
