About the Role:
As an LPN on our team, you will have the opportunity to work in a fast-paced and dynamic environment, providing direct patient care and support to our residents. Your responsibilities will include:
Providing and administering medications, treatments, and therapies as prescribed by physicians and as outlined in our nursing protocols
Conducting physical and vital sign assessments, documenting patient information, and reporting changes to the healthcare team
Developing and implementing individualized care plans in collaboration with the interdisciplinary team
Assisting with daily living activities, such as bathing, dressing, and grooming
Maintaining a safe and clean environment for our residents, adhering to infection control protocols and adhering to policies and procedures

Requirements:
Current LPN licensure in the state of Illinois
High school diploma or equivalent required; associate's degree or higher preferred
Minimum of 0 years of experience in a healthcare setting; prior experience as an LPN a plus
