Summary Of Position
The Education Specialist/Community Health Worker (CHW) is responsible for coordinating and implementing activities under the Dementia in Tribal Communities grant. This position leads the planning, development, and delivery of culturally responsive dementia education and training for community members, caregivers, and partner organizations. The Education Specialist develops educational materials, organizes outreach and training events, and supports community awareness efforts related to dementia, brain health, caregiver support, and other health issues affecting the target population. As a CHW, the Education Specialist serves as a trusted liaison between community and the healthcare system, helping to reduce barriers to services and strengthen partnerships across the tribal health and social service networks.

Summary Of Position
The Education Specialist/Community Health Worker (CHW) is responsible for coordinating and implementing activities under the Dementia in Tribal Communities grant. This position leads the planning, development, and delivery of culturally responsive dementia education and training for community members, caregivers, and partner organizations. The Education Specialist develops educational materials, organizes outreach and training events, and supports community awareness efforts related to dementia, brain health, caregiver support, and other health issues affecting the target population. As a CHW, the Education Specialist serves as a trusted liaison between community and the healthcare system, helping to reduce barriers to services and strengthen partnerships across the tribal health and social service networks.
Minimum Qualifications
- High School Diploma or related field with at least (1) year of experience in community outreach, health promotion, screening, or elder/caregiver support; or equivalent combination of education and experience.
- Direct experience in community health work, caregiving education, or elder health outreach is preferred. Knowledge of dementia, brain health, or caregiving support is high desirable, with willingness to participate in ongoing training.
- Valid New Mexico driver's license with the ability to meet Tribal liability insurance requirements.
- New Mexico Community Health Worker (CHW) certification, or the ability to obtain certification within 90 days of hire
- Must be 21 years or older,
- Must be able to satisfactorily pass a background check and new hire drug screen.
- Speaks and understands the Keres language is preferred.
- Excellent oral and written communication skills to focus efforts on meeting Santo Domingo Pueblo goals.
- Extended working hours, including evenings, weekends, and holidays may be required.
