Admissions Coordinator

Santa Fe Post Acute is looking for a motivated Admissions Coordinator to be the welcoming first point of contact for our residents and families. This role is vital to ensuring a smooth, compliant, and positive admissions experience from start to finish.

What You’ll Do

• Manage the full admissions process for new residents from inquiry to placement
• Collect, verify, and maintain admission documentation in compliance with HIPAA
• Coordinate with nursing, therapy, case management, and physicians to ensure timely admissions
• Educate residents and families on insurance benefits, including Medicare
• Maintain accurate medical records and update systems as needed
• Communicate clearly and compassionately with residents, families, and referral sources
• Support a smooth transition for residents to ensure a positive first impression

What We’re Looking For

• Prior admissions experience in a Skilled Nursing Facility (SNF) or ALF strongly preferred
• Solid understanding of HIPAA, medical records, and healthcare workflows
• Familiarity with Medicare and insurance verification processes
• Strong organizational and multitasking skills
• Excellent verbal and written communication skills
• Bilingual (Spanish/English) is a plus
• Team-oriented mindset with a patient-centered approach
