Are you an organized, proactive professional with a passion for making a difference in hospice care? We’re looking for a dedicated Patient Care Coordinator to ensure smooth operations and support compassionate care at our Pueblo Hospice.
What You’ll Do:
Coordinate Patient Care: Manage CNA schedules and patient caseloads, ensuring seamless daily visits.
Plan Ahead: Develop and update staffing plans weekly, monthly, and quarterly.
Support the Team: Address clinician needs, cover shifts, and handle administrative tasks with finesse.
Ensure Compliance: Collaborate with Medical Records and Compliance to maintain top-quality care aligned with care plans.
Grow and Adapt: Identify training needs and work with education departments to keep staff at their best.

This is a Full Time Benefited position. Monday - Friday, 8-5

Requirements
- High School Diploma and at least 1 year of office experience
- Scheduling Skills: 6 months of experience coordinating teams is a plus!
- Organizational Excellence: Stay on top of schedules, calls, and documentation
- Tech-Savvy: Proficiency in Excel, Word, Outlook, and typing 34 WPM
- Strong Communication: Connect effectively with staff and patients alike
Salary : $18 - $20
