Sangre de Cristo Community Care is looking for a Part Time Bachelor’s Level (Master’s preferred) Social Worker and Volunteer Coordinator to join our dynamic team in our Walsenburg or Trinidad, CO office. - Competitive wages. $24.50 -$32.00 per hour based on degree and experience

Responsibilities:
- Provides psychosocial support to patients and families by provided skilled social work invention
- Provides resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements
- Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.
- Plans, coordinates, and manages the volunteer program
- Supervisors and trains new volunteers
- Identifies needs for volunteers and assigning of volunteers to patients and families

Requirements
- A Bachelor’s Degree in social work, counseling, or related field from an accredited university (Master’s preferred)
- Must have a valid non-restricted driver’s license
- Bilingual in Spanish preferred
- Flexibility and availability to work a weekend schedule if needed.
- At least 1 year of healthcare experience preferred.
- Previous volunteer coordination preferred
Salary : $25 - $32
