What are the responsibilities and job description for the Intensive In Home Team Lead position at Sanford SHYAS?
S&H Youth And Adult Services in Sanford, NC is looking for one IIH- Intensive In Home Team Lead to join our team.
Responsibilities include:
- Responsible for providing assessment and therapy services for children who are eligible for IIH services and their families.
- Facilitating relationships and serving as a link between the company, parents, guardians, local agencies and the community
- Provides individual therapy for beneficiaries served by the team
- Coordinating and overseeing the initial and ongoing assessment activities
- Completing the initial development and ongoing revision of the PCP and ensuring its implementation
- Facilitates weekly team meetings and provides clinical expertise and guidance to the IIH members in the team’s interventions with the beneficiary
Qualifications
- QMHP Preferred
- Clinical License - licensed clinician ( LCAS, LPC, and LCSW)
- Minimum 2 years experience working with individuals with mental and/or behavioral health issues that include writing person-centered plans, conducting effective group therapy and providing mental health diagnoses
- Valid driver’s license, auto insurance and reliable transportation
