Discharge Planner - Medical Detox

Position: Discharge Planner

Location: Chantilly Virginia 

Work Environment: Medical Detox/ Residential Treatment Center 

Schedule: Monday to Friday  8:30am-5:00pm

Compensation: $18-24 Per Hour (dependent on experience) 

About The Role  

The Discharge Planner plays a vital role in ensuring clients transition smoothly from a Detox level of care to ongoing treatment and community-based resources. This position is responsible for discharge planning, developing aftercare summaries, coordinating follow-up care, and maintaining strong relationships with referents, treatment centers, and community providers 

Key Responsibilities: 

• Lead discharge planning, including aftercare coordination and documentation of discharge summaries. 
• Collaborate with clinical teams to assess client needs and develop individualized aftercare plans. 
• Serve as a liaison between clients, families, referral agencies, and community providers to ensure continuity of care. 
• Participate in clinical team meetings to discuss treatment progress, challenges, and discharge planning strategies. 
• Adhere to policies and procedures to ensure compliance with regulatory and quality standards. 
• Work closely with admissions, clinical, and outreach teams to maintain strong communication and care coordination. 
• Educate clients and families on available aftercare options, addressing potential barriers to continued treatment. 
• Maintain organized and accurate records of all discharge planning activities. 

Education & Experience Requirements: 

• Education: Bachelor’s degree in behavioral health science (Social Work, Psychology, Marriage and Family Therapy, or related field) or Education preferred. 
• Experience: 
  • Previous experience in substance abuse treatment required. 
  • Experience in a detox or residential treatment facility preferred. 
  • Discharge planning experience highly desirable. 
  • Business development experience is a plus. 
• Skills / Capabilities:  
  • Clinical Coordination: Ability to assess client needs, coordinate treatment team input, and develop effective aftercare plans. 
  • Communication & Collaboration: Strong verbal and written communication skills to engage with clients, families, and external partners. 
  • Crisis Management & Problem-Solving: Ability to navigate complex situations, address challenges, and provide appropriate aftercare solutions. 
  • Organizational Skills: Detail-oriented with strong documentation, scheduling, and follow-up abilities. 
  • Public Relations & Advocacy: Capable of building relationships with community providers, advocating for client needs, and ensuring continuity of care.
