- Manage incoming referrals and respond in a timely manner
- Coordinate admissions from referral to admission, ensuring a smooth transition for residents and families
- Communicate with hospitals, case managers, and internal teams to gather necessary documentation
- Verify insurance and assist with eligibility when needed
- Work closely with the Admissions Director and clinical team to review and accept appropriate referrals
- Track and follow up on all leads to maximize census growth
- Provide a strong first impression to families and referral sources
- Experience in admissions, intake, or healthcare coordination preferred
- SNF or healthcare experience strongly preferred
- Strong communication and organizational skills
- Ability to multitask and work in a fast-paced environment
- Comfortable working with insurance verification and documentation
- Team-oriented with a sense of urgency
