Position Description
The Client Care Coordinator works to enhance the effectiveness in achieving Sandcastle’s goals of the high-quality care and customer service while demonstrating compliance with all regulatory agency standards. The Client Care Coordinator works directly with the client, the client’s family, and the care team. Each clients’ care team consists of the Scheduling team, the caregivers, and any 3rd party directly involved with the care of the client.

The Client Care Coordinator works directly with the client, the client’s family, and the care team. Each clients’ care team consists of the Scheduling team, the caregivers, and any 3rd party directly involved with the care of the client.
Position Responsibilities
- Collaborates with the care team regarding a client’s plan of care.
- Serve as an educator and resource for each client (and family) in managing their own care in order to ensure optimal quality of care.
- Identifies, evaluates, and acts to resolve any potential barriers to delivery of care.
- Arrange for additional care appropriately according to needs of client.
- Follow up with scheduling daily to ensure accountability.
- Ensure paperwork is compliant with state guidelines.
- Include HR Coordinator in all caregiver issues.
- Bring all compliance concerns to the office manager in a timely manner.
- Other duties as assigned.
